How to ask the question why I was not included on the list?

Why were you left off of the list?

There is a saying that says something like: "The simplest answer is usually the right one." Here is what I mean:

The most likely reason is just a simple oversight. When you are dealing with a committee as you refer to in your question, it is easy to forget someone. It is likely just an honest mistake and done unintentionally.

If it were me, I would approach someone in the committee that I had a good relationship with and ask them if there was a reason that I was left off of the list. They would either tell me if there was a reason I was left off, or they would be shocked about the oversight.

If it was an oversight, they would probably go to the person in charge of the committee and point it out for future correction.

Start with the person on the committee that you have the best relationship with and go from there. Good luck!
